ABSTRACT

Objective@#To investigate the value of pulmonary ventilation/perfusion (V/Q) SPECT imaging in fibrosing mediastinitis (FM).@*Methods@#From January 2015 to July 2018, 14 FM patients (6 males, 8 females, average age 74 years) who underwent V/Q SPECT imaging in China-Japan Friendship Hospital were retrospectively studied. The data of V/Q SPECT imaging were analyzed and the defect extent was classified as mild (<20%), moderate (20%-50%) and severe (>50%) according to the percentage of pulmonary perfusion and ventilation defect in total lung volume (%). χ2 test was used to analyze the incidence rates of the impaired blood perfusion of each lung lobe.@*Results@#According to the results of Q SPECT imaging, all 14 patients had impaired blood perfusion in the superior lobe of left lung, and the number of patients with/without impaired blood perfusion in the inferior lobe of left lung, superior lobe of right lung, middle lobe of right lung, inferior lobe of right lung were 11/3, 14/0, 13/1, 12/2, respectively. The incidence rates of impaired blood perfusion in different lobes were not significantly different (χ2=6.198, P=0.185). The range of lung perfusion defect was mild in 1, moderate in 7 and severe in 6 patients. The incidence rates of impaired blood ventilation in different lobes were not significantly different (χ2=1.587, P=0.811). The range of lung ventilation defeat was mild in 11, moderate in 2 and severe in 1 patients. The defect extent of lung segments and subsegments in Q SPECT imaging and V SPECT imaging were 119 and 41, respectively (χ2=28.42, P<0.05). There was more defect in Q SPECT imaging, but both methods showed segmental or subsegmental distribution.@*Conclusions@#The defect of V/Q SPECT imaging in FM patients is segmental in distribution, and more is observed in Q SPECT imaging. Pulmonary V/Q SPECT imaging can evaluate the defect range and extent of pulmonary perfusion and ventilation in FM patients.

ABSTRACT

Objective To study the influence of excessive iodine intake on the 99Tcm-pertechnetate uptake function of thyroid and explore the related mechanism. Methods A total of 150 male Wistar rats were divided into normal iodine group (NI group) and 4 high iodine (HI) groups (10HI,50HI,100HI and 500HI group) with completely random method. Each group included 30 rats.Rats in HI groups were intragastrically administrated with different concentrations of KIO3,and rats in NI group were administrated with the same amount of distilled water. After a 7 d HI diet, each group was randomly divided into 3 subgroups:7 d group (HI diet for 7 d),10 d group (HI diet for 7 d + limiting iodine for 3 d) and 14 d group (HI diet for 7 d + limiting iodine for 7 d).All rats were given 1.85 MBq 99TcmO4-,then the thyroid glands were isolated and weighed 20 min post-injection.The radioactive counts were measured and the thyroid uptake percentage activity of injected dose per gram of tissue (%ID/g) of 99TcmO4-for each sample was calculated.Sodium-iodide symporter (NIS) expression level was analyzed by Western blot.One-way analysis of variance or Welch test was used to compare the differences among groups, and Spearman rank correlation analysis was also used. Results At the same observation time, the %ID/g of 99TcmO4-and NIS expression among different HI groups were statistically significant (F values:48.825-119.219,all P<0.01).In NI and 10 HI groups,%ID/g of 99TcmO4-and NIS expression were not significantly different among different time subgroups (F values:0.090-4.753,all P>0.05).Significant differences of % ID/g of 99TcmO4-and NIS expression were found among different time subgroups in 50HI,100HI and 500HI groups (F values:10.442-90.408,all P<0.01).Spearman correlation analysis showed that the concentration of iodine correlated negatively with %ID/g of 99TcmO4-and NIS expression (rs values:-0.853 and-0.950,both P<0.01),while the %ID/g of 99TcmO4-and NIS expression was positively correlated (rs =0.925,P<0.01).Conclusions Excessive iodine intake can inhibit thyroid uptake of 99TcmO4-in rats, which is associated with the down-regulation of NIS expression. The degree of inhibition correlates positively with iodine concentration, and correlates negatively with NIS expression. The degree of recovery of NIS and %ID/g of 99TcmO4-may relate to the time of limiting the iodine intake.

ABSTRACT

Objective To develop a diagnostic model based on deep neural network for intelligent discrimination of thyroid function. Methods A total of 1616 patients ( 283 males, 1333 females, average age:52 years) who underwent thyroid imaging between May 2016 and June 2018 were selected. According to the clinical diagnosis, the 1616 cases included 299 normal thyroid cases, 876 hyperthyroidism cases and 441 hypothyroidism cases. Feature extraction and learning training were performed on 1000 training set sam-ples by two deep neural network models ( AlexNet;deep convolution generative adversarial networks ( DCGAN) ) using deep learning algorithm. Performance verifications were implemented on 616 test set samples. The con-sistency between the verification results of the two models and the clinical diagnosis was analyzed by Kappa test. Meanwhile, the time advantage of the intelligent diagnosis models was analyzed. Results The average diagnostic time of AlexNet model was 1 s/case, and the classification accuracy for normal thyroid, hyperthy-roidism, hypothyroidism were 82.29%(79/96), 94.62%(369/390), 100%(130/130), respectively. The Kappa value between results of AlexNet model and clinical diagnosis was 0.886 ( P<0.05) . The average di-agnostic time of DCGAN model was 1 s/case, and the classification accuracy for normal thyroid, hyperthy-roidism, hypothyroidism were 85.42%(82/96), 95.64%(373/390), 99.23%(129/130), respectively. The Kappa value between results of DCGAN model and clinical diagnosis was 0.904 ( P<0.05) . Conclusion The deep neural network intelligent diagnosis model can quickly determine the functional status of thyroid gland in thyroid imaging, and it has a high recognition accuracy, thus providing a new method for thyroid image review.

ABSTRACT

BACKGROUND: Postoperative routine radioiodine (RAI) treatment is currently debated for patients with low-risk differentiated thyroid carcinoma (DTC) patients. If performed, a low 131I activity (i.e., 1 to 2 GBq) is recommended with the aim to ablate thyroid remnant and facilitate subsequent follow-up by thyroglobulin measurement. The purpose of this study was to evaluate the relationship between postsurgical technetium-99m (99mTc)-pertechnetate scintigraphy and the rate of successful remnant ablation after low activity radioiodine ablation in patients with DTC. METHODS: Enrolled were 193 patients with low risk DTC who underwent total thyroidectomy and RAI ablation with a fixed 1.1 GBq activity of 131I. 99mTc-pertechnetate scans were done and thyrotropin stimulated thyroglobulin (sTg) levels measured just before ablation. Ablation effectiveness was assessed 6 to 12 months later by sTg measurement, neck ultrasound and diagnostic whole body scan. RESULTS: A negative 99mTc-perthecnetate scans was the best predictor of successful ablation (P<0.001) followed by preablative sTg levels <0.8 ng/mL (P=0.008) and 99mTc-pertechnetate uptake rate values <0.9% (P=0.065). Neither sex nor age of the patient at the time of ablation or tumor histology and size showed a significant association with the rate of successful ablation. CONCLUSION: The 99mTc-pertechnetate scintigraphy is a simple and feasible tool to predict effectiveness of low activity 131I thyroid to ablate thyroid remnants in patients with DTC.

ABSTRACT

Objective To validate the quantitative accuracy of 99Tcm-pertechnetate (99 Tcm O4-)SPECT/CT imaging and its influencing factors (sphere size, radioactive concentration, reconstruction methods). Methods National Electrical Manufacturers Association (NEMA) International Electrotechnical Commission (IEC) body phantom with 99TcmO4-solution filled in 6 spheres (diameter:10,13,17,22,28,37 mm) and 1 cylinder (target/background ratio:10∶1) was used to assess the accuracy of uptake quantification.SPECT/CT was used for imaging data acquisition, and volume of interest (VOI) was drawn after the images being corrected and reconstructed. The radioactive concentrations of 6 spheres were measured and compared with the true radioactive concentrations, and quantitative accuracy of each sphere was calculated. The experiment was repeated for 12 times with the same method and different radioactive concentrations.Ordered-subsets expectation maximization (OSEM) reconstruction was performed with 2 or 5 iterations to process imaging data.CT VOI and nuclear medicine (NM) threshold methods were used for the delineation of VOI.Accuracy was compared by paired t test. Results The NM thresholds to get calculated volume close to the true volume were determined to be 40%,40%,50%,70%,80%,90% for 6 spheres with diameters of 37,28,22,17,13,10 mm,respectively.Comparing 2 methods of VOI delineation (CT VOI and NM threshold),only the accuracy for 17 mm sphere was significantly different (t=5.89,P<0.01).Five iterations group showed higher accuracy than 2 iterations group (t values:3.19-6.92,all P<0.05).The average accuracies under 12 different radioactive concentrations were 102%,95%,80%,66%,27%,20%,respectively,for spheres in order from large size to small size. Conclusion The sphere volume and reconstruction method have effect on quantitative accuracy of 99TcmO4-SPECT/CT imaging, and spheres smaller than 17 mm is also significantly affected by partial volume effect.

ABSTRACT

Objective To evaluate the damage of the salivary gland in Sjogren's syndrome (SS)patients with normal Young's modulus value (YMV) using salivary gland dynamic imaging (SGDI).Methods A total of 78 parotid glands and submandibular glands of SS patients and 100 parotid glands and submandibular glands of healthy people were collected in this retrospective study. The difference of YMV between 2 groups was analyzed with two-sample t test;receiver operating characteristic (ROC) curves of YMV were drawn to calculate the area under carve (AUC) and cut-off values.SGDI was used to observe the uptake ratio of 15 min (UR15) value in the SS salivary glands with normal YMV.Results The YMV of left parotid/submandibular glands and right parotid/submandibular glands were not statistically different in both SS patients and healthy people (t values:from-1.192 to-0.404,all P>0.05).The YMV of parotid/submandibular glands in SS patients was higher than that in healthy people:(15.39±8.08) vs (9.33±2.21) kPa,(14.35±5.33) vs (9.18±2.36) kPa (t values:7.169 and 8.662,both P<0.05).ROC curves showed that the AUC of parotid glands and submandibular glands were 0.78 and 0.83.As the cut-off values of YMV were 10.85 kPa and 10.95 kPa,the sensitivity were 69.23% (54/78) and 73.08% (57/78),the specificity were 77.00% (77/100) and 83.00% (83/100),the positive predictive value were 70.13% (54/77) and 77.03% (57/74),the negative predictive value were 76.24% (77/101) and 79.81% (83/104),respectively.There was no statistically difference between YMV and UR15 for evaluating the SS parotid glands and submandibular glands (x2 values:0.963 and 0.896,both P>0.05).The UR15 value of 78 SS parotid glands and submandibular glands were 1.42±0.97 and 1.19±0.83,respectively.About 58.33% (14/24) parotid glands with normal YMV (<10.85 kPa) and 76.19% (16/21) submandibular glands with normal YMV (<10.95 kPa)had reduced cellular uptake in SS patients. Conclusions Parotid glands and submandibular glands in SS patients are damaged, and the tissue hardness is increased.SGDI can discover the cellular damage in SS salivary glands with normal YMV.

ABSTRACT

PURPOSE: The biodistribution of sodium pertechnetate, the most used radiopharmaceutical in nuclear medicine, has not been studied in details after bariatric surgery. The objective was to investigate the effect of Roux-en-Y gastric bypass (RYGB) on biodistribution of sodium pertechnetate (Na99mTc-) in organs and tissues of rats. METHODS: Twelve rats were randomly divided into two groups of 6 animals each. The RYGB group rats were submitted to the Roux-en-Y gastric bypass and the control group rats were not operated. After 15 days, all rats were injected with 0.1mL of Na99mTc- via orbital plexus with average radioactivity of 0.66 MBq. After 30 minutes, liver, stomach, thyroid, heart, lung, kidney and femur samples were harvested, weighed and percentage of radioactivity per gram ( percentATI/g) of each organ was determined by gama counter Wizard Perkin-Elmer. We applied the Student t test for statistical analysis, considering p<0.05 as significant. RESULTS: Significant reduction in mean percentATI/g was observed in the liver, stomach and femur in the RYGB group animals, compared with the control group rats (p<0.05). In other organs no significant difference in percentATI/g was observed between the two groups. CONCLUSION: This work contributes to the knowledge that the bariatric surgery RYGB modifies the pattern of biodistribution of Na99mTc-.

ABSTRACT

BACKGROUND/AIMS: Technetium-99m ((99m)Tc)-pertechnetate single photon emission computed tomography (SPECT) allows non-invasive measurement of gastric accommodation (GA) and has been reported to provide the results comparable to the gold standard barostat. The reproducibility of the test when it is performed at different time of the day is not known. The aim of this study was to assess the reproducibility of GA measurement by (99m)Tc-pertechnetate SPECT at the same and different time of the day. METHODS: Three GA studies were performed in each of 20 healthy volunteers (9 males, age 35 +/- 9 years), 2 studies in the morning and 1 study in the afternoon. Each study was performed on separate days at least 3 days apart, in random order. The fasting gastric scintigraphy images were acquired using SPECT at 30 minutes after iv (99m)Tc-pertechnetate. Then, the scintigraphic images were acquired 50 minutes after liquid meal (250 mL, Ensure(R)) ingestions. Gastric volumes were calculated from transaxial gastric images using Image J software. The GA was defined as the maximum postprandial gastric volume minus fasting gastric volume. RESULTS: GA measurement by SPECT show good reproducibility at both the same and different time of the day. The intraclass correlation coefficients of GA results for the same time, the first different time and the second different time studies were 0.681, 0.630 and 0.774, respectively, and the intra-subject coefficients of variation were 13.9%, 14.1% and 8.7%, respectively. CONCLUSIONS: GA measurement by (99m)Tc-pertechnetate SPECT shows good reproducibility at both the same and different time of the day. This result suggests that the schedule time does not appear to extremely affect the gastric accommodation value.

ABSTRACT

PURPOSE: To asses the dissemination of bacteria labeled with technetium-99m (99mTc) from peritoneal cavity after different surgical procedures. METHODS: Bacteria of the Escherichia coli species labeled with 99mTc were used in a concentration of 10(8) units of colony-makers for ml (UFC/ml) and 1ml was inoculated through intra-peritoneal via. Forty-eight rats were divided into four groups: control, laparotomy, pneumoperitoneum with 10mmHg and pneumoperitoneum with 20mmHg of CO2. Procedures were performed 20 min after injection of the inoculum and lasted 30 min. Animals were sacrificed after six hours (Group 1) and 24 hours (Group 2). Samples of blood, liver and spleen were collected for radioactivity counting. RESULTS: After six hours, indirect detection of the bacteria in different organs was uniform in all groups. After 24 hours, a larger detection of technetium was observed in the livers of animals of the group insufflated with 20mmHg of CO2, when compared with those of control group (p<0.01). The other groups did not present statistically significant variations. CONCLUSIONS: The use of a higher intra-abdominal pressure was associated with a higher bacterial dissemination to the liver. The application of lower intra-abdominal pressures may be associated with a lower dissemination of the infectious status during laparoscopic approach of peritonitis status.

ABSTRACT

PURPOSE: To evaluate the biodistribution of sodium pertecnetate (Na99mTcO4) in organs and tissues, the morphometry of remnant intestinal mucosa and ponderal evolution in rats subjected to massive resection of the small intestine. METHODS: Twenty-one Wistar rats were randomly divided into three groups of 7 animals each. The short bowel (SB) group was subjected to massive resection of the small intestine; the control group (C) rats were not operated on, and soft intestinal handling was performed in sham rats. The animals were weighed weekly. On the 30th postoperative day, 0.l mL of Na99mTcO4, with mean activity of 0.66 MBq was injected intravenously into the orbital plexus. After 30 minutes, the rats were killed with an overdose of anesthetic, and fragments of the liver, spleen, pancreas, stomach, duodenum, small intestine, thyroid, lung, heart, kidney, bladder, muscle, femur and brain were harvested. The biopsies were washed with 0.9 percent NaCl.,The radioactivity was counted using Gama Counter WizardTM 1470, PerkinElmer. The percentage of radioactivity per gram of tissue ( percentATI/g) was calculated. Biopsies of the remaining jejunum were analysed by HE staining to obtain mucosal thickness. Analysis of variance (ANOVA) and the Tukey test for multiple comparisons were used, considering p<0.05 as significant. RESULTS: There were no significant differences in percentATI/g of the Na99mTcO4 in the organs of the groups studied (p>0.05). An increase in the weight of the SB rats was observed after the second postoperative week. The jejunal mucosal thickness of the SB rats was significantly greater than that of C and sham rats (p<0.05). CONCLUSION: In rats with experimentally-produced short bowel syndrome, an adaptive response by the intestinal mucosa reduced weight loss. The biodistribution of Na99mTcO4 was not affected by massive intestinal resection, suggesting that short bowel syndrome is not the cause of misleading interpretation, if...
